You can embed and display the MS Forms form directly on Modern SharePoint page using one of the web parts below.
Microsoft Forms web part to display just the content on the form:
- Copy the whole URL of the form
- Add the Microsoft Forms web part to your page and insert the URL from above.
Embed web part to display the form including the theme:
- Under the Share button, when you click on the
</>icon, you’ll get a code to copy.
